①供应商为具有独立承担民事责任能力的法人或其他组织或自然人，并出具营业执照（事业法人证）或证明文件或自然人的身份证明；

②提供具有财务审计资质单位出具的2019（或2020）年度财务报告或开标前六个月内其基本账户银行出具的资信证明（附开户许可证）或政府采购信用担保机构出具的投标担保函；

③出具具有履行合同所必需的设备和专业技术能力的声明；

④具有依法缴纳税收和社会保障资金的良好记录（提供开标前六个月内任意一个月的缴纳证明）；
